This lack of information is particularly challenging for individuals living with chronic diseases during the stages of pregnancy and breastfeeding, as it does not all allow for an informed dialogue with their healthcare providers (HCPs) about optimal management and treatment of their diseases at those times.</p><p>As a member of the E21 Expert Working Group (EWG), a part of The International Council for Harmonisation of Technical Requirements for Pharmaceuticals for Human Use (ICH), I am proud to be part of efforts to change this paradigm. It is high time all individuals, regardless of reproductive status, have access to the most effective treatments and information to make better decisions.</p><p>The E21 EWG was formed to create a new guideline for the inclusion of pregnant and breastfeeding individuals in clinical trials (CTs) – one that harmonises strategies and methodologies to ensure the appropriate inclusion and/or retention of pregnant and breastfeeding individuals in CTs.</p><p>This new efficacy guideline will provide a globally accepted framework and best practices to enable inclusion and/or retention of pregnant and breastfeeding individuals in clinical trials – including practical guidance for planning trials that include pregnant and breastfeeding individuals, as well as comprehensive communication and stakeholder engagement plans.</p><p>A global-level agreement is needed to improve the situation for pregnant and breastfeeding individuals who need medicines, and to be inclusive of all patient populations. This will help reduce the risk of prescribing medicinal products without adequate evidence in these patient populations. The guideline, based on globally accepted principles, will be implementable worldwide, thereby promoting inclusive patient care and ensuring ethical data generation in global trials.</p><p>This work is more than just a professional responsibility; it is a moral imperative. ֳ is committed to improving the care for pregnant and breastfeeding individuals of childbearing age by addressing any gaps in knowledge. In the ever-evolving healthcare landscape, Artificial Intelligence (AI) and the digitalization of healthcare, is set to create new standards and advancements in care for people living with epilepsy and rare epileptic syndromes. This digitalization is revolutionizing the way we understand and approach these complex neurological disorders, with AI helping us to push the boundaries of healthcare and drug development by enabling us to support more precise diagnoses, detect patterns and trends, and aid disease prevention.<br><br>However, the successful integration of AI into healthcare relies heavily on a foundation of trust – trust from the public, medical professionals, and caregivers. We understand that this trust has improved drastically over the past few years, with events like COVID-19 necessitating remote digital healthcare, but there remains a distrust in the public domain. The main questions we are being asked are “<em>Where is my data being used?</em>”, “<em>Who is looking at my data?</em>” and “<em>How will my data be used?</em>”<br><br>Because of this, it is our role as experts in neurology to address these questions and be transparent. While the challenge of trust is not specific to ֳ but industry-wide, we are continuously working to build that trust with patients, caregivers, and medical professionals.<br>Collaboration and partnerships are an essential way to build trust, and we know that we cannot do this alone. We must turn to expert digital partners, such as Microsoft, to support us on this journey. The continuous work we do together establishes a strong foundation for the future and landscape of neurology.<br><br>One example of how we’re using AI is the development of wearable technology – being able to track movement disorders not only helps patients but also increases our own understanding of how they manage their condition. The development of this technology also allows us to monitor patient data in real-time but has also enabled us to engage with a larger, more diverse patient pool within remote clinical trials. We can collect more data and ensure we secure better equity around patients, effectively reaching underserved populations.<br><br>Another area we are currently exploring is the use of ‘<em>Digital Twins</em>’ - a digital copy - to accurately reflect a physical human brain. The use of this modeling technology could assist in identifying the pattern of symptomatic behaviors such as seizures and, in principle, help medical professionals to individualize treatment based on the simulated behavior to predict how a specific patient might react to treatment.<br><br>Given the digital landscape is evolving rapidly, we must keep up with the pace and constantly look at ways we can digitalize our offering to secure a better outcome for our patients. We are showcasing 29 abstracts, including 4 late breakers, covering early research, clinical, health economic, and demographic studies in epilepsy.<br> <br> It’s a truly broad data set, providing new insights in areas such as Dravet syndrome, Lennox-Gastaut syndrome, epilepsy and pregnancy, focal-onset seizures, SUDEP, and for individuals who experience prolonged seizures, all with a focus on elevating epilepsy care today. We also touch on potential treatments of tomorrow, presenting some of our early discovery research work in our ongoing pursuit of treatments that could one day alleviate the underlying causes of epilepsies. We also explore factors contributing to health disparities amongst those living with epilepsy, and what can be done to bring about change.<br> <br> This year, we are also excited to be partnering with the AES as the exclusive sponsor of components of their Diversity, Equity, and Inclusion (DE&amp;I) initiatives at the AES 2023 Annual Meeting. This collaboration signifies our shared values and commitment to fostering an environment of inclusivity, diversity, equity, and excellence. Through this collaboration, ֳ supports AES's aim to provide essential career support for both current and emerging epilepsy scientists and clinicians who belong to historically underrepresented groups and underserved populations. More than ever, identifying how to enhance the healthcare experience of these women is a priority.</p> <p>With that in mind, ֳ partnered with patients representing multiple chronic diseases and specialist healthcare professionals (HCPs) spanning rheumatology, neurology, dermatology, psychology, and obstetric medicine across the globe. Together, we discussed how we can address challenges faced by women of childbearing age living with various chronic diseases and proposed comprehensive solutions.</p> <p>Four primary unmet needs in the healthcare journey of women of childbearing age living with chronic disease were identified: a lack of patient support, insufficient specialist discussions, suboptimal coordination of the care pathway, and the need to better empower women.</p> <p>To face those barriers, a powerful collaboration between patients and healthcare professionals across multiple therapeutic areas is the cornerstone of progress. Together, their combined efforts create a synergy that goes beyond individual therapeutic area, making the journey towards solutions for chronic conditions in WoCBA far more impactful. By actively working together, we not only enhance our understanding but also pave the way for tailored and effective solutions, ultimately elevating the standard of WoCBA patient care. In the dynamic workshop environment, participants pinpointed critical steps, including the development of comprehensive information resources for HCPs and patients. The focus extended to strengthening collaboration among specialist HCPs through a multidisciplinary team approach and the establishment of patient peer support groups.</p> <p>At ֳ, we are committed to continue collaborating with expert groups, patients, and HCPs. Patients or people living with an epilepsy diagnosis, and their caregivers, are the true experts of their condition and it is their narratives that shape our scientific journey.&nbsp;<br> <br> So, before we go into any clinical trial, we make sure we have an in-depth understanding of the persons experience of living with the disease to better understand how a medicine is going to help the patients and their families.&nbsp;<br> <br> In the evolving landscape of epilepsy research, a profound shift is underway at ֳ. We are delving even deeper into the multi-faceted experiences of people living with a range epilepsies and rare syndromes. We also aspire, in epilepsy, to bring disease modifying medications to improve patients’ lives with potential for curative therapies.&nbsp;<br> <br> We are looking beyond the seizures at the other co-morbidities that add to the burden of disease to explore the holistic impact of certain medications on the daily lives of people living with epilepsy. We recognize the uniqueness of each epilepsy journey, acknowledging that no two experiences are identical, and see this as a challenge that we will overcome together with the patient communities.&nbsp;<br> <br> Targeting the true unmet needs of those dealing with seizures can be challenging. For starters, there are different racial, ethnic, age groups, and socioeconomic backgrounds that need to be factored into the patient experience. As part of our research, we focus on areas of unmet need, in the hope to find inclusive representation of refractory patients and those living with rare syndromes. As we approach medicines in development and commercialization, we work to meet the needs of the varying, low-prevalence patient populations.<br> <br> For instance, during our post-trial commercialization work, we secured insights from varying ethnic and socioeconomic backgrounds, including single and married caregivers to better understand logistical challenges of daily life. To bridge the gap even further, we collaborate with patient advocacy organizations to be as inclusive as possible. However, we acknowledge there’s a natural selection of advocates who have the bandwidth to engage with patient organizations; they tend to be in a higher socio-demographic bracket, which can bias our research.&nbsp;<br> <br> To combat this, we must check our own unconscious bias and diversify who and where we conduct our research. We are making a conscious effort to ensure these groups are not overlooked by bringing services and accessible information to hard-to-reach communities. A small step begins with ensuring the language we use is universally understood. Patient and caregiver facing materials that ֳ produces for the patient and caregiver community need to be understood by an eight-year-old. I have always found my motivation in empowering people to manage their health, so their life can be the focus, not their disease. At ֳ, we consider each person’s journey to help them achieve their own goals and live their best possible life.<br> <br> Part of what I’ve found makes ֳ unique is the robust research, both quantitative and qualitative, that goes into understanding the patient journey and needs, and applying these insights in every stage of the clinical development and commercialization process. We talk with people living with psoriasis and listen to what they experience, which helps us appreciate the impact it can have on their physical and emotional well-being.<br> <br> At the surface, psoriasis is one of the most common autoimmune disorders, affecting approximately 7.5 million people in the U.S. alone. It causes rapid build-up of extra skin cells that form thick, silvery scales and itchy, dry, red patches. But when you dig deeper, you learn that psoriasis is more than a skin disease.<br> <br> We have heard that after receiving a psoriasis diagnosis, many people feel forcibly displaced from their sense of self. People living with psoriasis often report negative mental health and social well-being, and through an online survey of 500 adult patients in the U.S., more than 90% of people with a moderate-to-severe form of the condition feel their psoriasis has a significant impact on their quality of life. They begin their care journey and oftentimes feel there is no clear destination or relief. More than half of patients being treated for moderate-to-severe plaque psoriasis don’t feel that they are achieving clear or almost clear skin and, more importantly, many of these patients do not believe this to be a realistic treatment goal. &nbsp;People living with psoriasis deserve better.<br> <br> At ֳ, we refuse to accept the conventional expectations for skin clearance. We know that increased levels of skin clearance are correlated with improvements in quality of life. These differences are further complicated for a woman diagnosed with epilepsy. She has a different experience living with her disease than a man does.&nbsp;<br> <br> Women need to consider their epilepsy treatments while preparing for the health of their unborn. They may be persuaded to undergo a C-section, and they may experience anxiety and fatigue after birth that can impact the frequency of seizures. Safety questions about breastfeeding may arise, along with so many other questions that a woman living with epilepsy must consider. Adding to these countless worries is the stress of finding accurate and relevant information during these pivotal and deeply personal moments. &nbsp;<br> <br> The simple, but complex question of why is epilepsy different for women was the focus of a recent<span style="color:#001489;"> </span><a href="https://www.ibe-epilepsy.org/" style="color:#001489;"><span style="color:#001489;">International Bureau for Epilepsy</span></a> (IBE) event entitled ‘<a href="https://www.ibe-epilepsy.org/why-is-epilepsy-different-for-women-webinar-replay/" style="color:#001489;"><span style="color:#001489;">Women &amp; Epilepsy</span></a>’, in which I had the privilege of participating. As part of a project by IBE to empower <a href="https://womenandepilepsy.org/" style="color:#001489;"><span style="color:#001489;">women with epilepsy</span></a>, experts from various fields were joined by women with the disease to share and discuss their lived experience of epilepsy. They shared clinical and social perspectives on how epilepsy has a unique and significant impact on women, especially once they reach childbearing age.<br> <br> Neurologists, psychologists, researchers, human rights experts, industry colleagues and, most importantly, women living with epilepsy, who are experienced experts in their disease, underscored:</p> <ul> <li><a href="/Patients/magazine/detail/article/Clinical-Trials-Day-2023-Strategic-Approaches-in-Pregnancy-and-Breastfeeding-Time-for-a-Paradigm-Shift" style="color:#001489;"><span style="color:#001489;">The clinical and ethical imperative to generate more and better standardized data to inform shared decision making on the use of medication during pregnancy and breastfeeding</span>.</a></li> <li>The need for intersectional solutions that go beyond medicines and include policy interventions and psycho-social support.</li> <li>The importance of collaboration across the medical, academic, advocacy, and private sector communities to drive patient-centered change.</li> <li>The need for a case team to enable effective communication between a woman’s neurologist or epileptologist team and gynecology team throughout pregnancy and maternity.</li> <li>The lived experiences of women living with epilepsy must be foundational to any action taken.</li> </ul> <p>ֳ is working to advance science for women of childbearing age with chronic diseases and to empower them to make shared decisions about their healthcare. Our effort in addressing knowledge gaps in the care of women of childbearing age began in rheumatology, and we are now embedding an emphasis on women of childbearing age across our current and potential future medicines.&nbsp;<br> <br> My ֳ colleagues and I are seeking to better understand and apply insights shared by people living with epilepsy and rare syndromes to bring meaningful change to their lives. In 2020, we conducted a social listening project that tracked more than 200,000 public social media posts, across six countries in the European Union and United States, over the course of a year. This listening initiative explored two key questions:</p> <ul> <li>What is the motherhood journey experience of women living with epilepsy?</li> <li>What are the unmet needs of women living with epilepsy?&nbsp;</li> </ul> <p>Based on the gathered insights, we convened a multidisciplinary community of patients, patient group representatives, and physicians from different therapeutic areas.&nbsp;<br> <br> By convening this community of experts, along with partnerships with clinical teams, <a href="/patients/Women-of-Childbearing-Age" style="color:#001489;"><span style="color:#001489;">ֳ strives to address knowledge gaps in the care of women of childbearing age</span></a> through the provision of validated evidence and quality disease management tools.&nbsp;<br> <br> It is fearless listening, continued collaboration, and a bias for action that will bring a paradigm shift toward protecting women through research — and not from research. Furthermore, this research is just one component in meeting the unique physical, emotional, and social needs of women of childbearing age living with epilepsy. &nbsp;<br> <br> Mothers, daughters, sisters – all women – should feel confident to request the information they need to make shared decisions about their treatment options before, during and after pregnancy.
