By analyzing real-world treatment patterns, RWE provides deeper insights into the effectiveness of osteoporosis treatments. Unlike clinical trials, which operate under controlled conditions, RWE captures the realities of routine clinical practice. &nbsp;</p><p>Worldwide, osteoporosis is estimated to cause fractures in a staggering one in three women over the age of 50, and in women over 45 years of age, osteoporosis accounts for more days in hospital than many other diseases, including diabetes, myocardial infarction and breast cancer.</p><p>With 71% of patients who sustain fractures related to osteoporosis remaining undiagnosed and untreated, data around treatment journeys is essential to help close the gap. A comprehensive picture of how patients experience care, manage their condition and navigate healthcare systems is lacking.</p><p><em>Highlighting the impact of RWE at WCO-IOF-ESCEO</em></p><p>At the 25th WCO-IOF-ESCEO Congress in Rome, Italy, ½ðºÌÓéÀÖ³Ç, its partner, Amgen and investigators from supported investigator-initiated studies, are presenting eight abstracts, further building the body of RWE that can be used to elevate care for those at high risk of fracture. &nbsp;</p><p>These data will allow us to better characterize patients at high fracture risk and get a clearer picture of their treatment journeys. These insights are guiding how we support clinicians to prioritize early, targeted interventions for patients most at risk of fracture. &nbsp;</p><p>At ½ðºÌÓéÀÖ³Ç, it is our mission that everyone with osteoporosis at very high fracture risk (VHFR) receives the appropriate investigation and intervention that they need, with the aim to create a world free of fragility fractures.</p><p>We are fully embracing the opportunity that RWE brings, generating data that deepens understanding of VHFR care and informs better treatment decisions. Around the world, the treatment gap in osteoporosis continues to be as pervasive as it is affecting; in an international survey last year, the International Osteoporosis Foundation (IOF) revealed that almost one in two women after the age of 50 and over experienced a fracture. Paradoxically, bone health – and the chance to identify and treat the underlying osteoporosis – was neglected during medical check-ups. The ramifications of this missed opportunity are huge.</p><p>Why does this matter? The impact on daily life for women with osteoporosis should not be underestimated, with fractures marked by excruciating pain, immobilization, and prolonged hospitalization, robbing them of their independence. This can have a significant impact on their well-being, leaving women feeling anxious and isolated. The lack of appropriate intervention at this crucial trigger point of the patient journey places a staggering burden on healthcare systems, with the cost of fractures in the EU anticipated to hit 77 billion EUR by 2050. &nbsp;</p><p>In the past year, we have made great progress in elevating secondary fracture prevention on government agendas – for example, <a href="https://pharmaphorum.com/patients/how-post-fracture-care-programmes-such-fracture-liaison-services-can-bridge-treatment-gap" target="_blank">the success in Japan following its reimbursement scheme for Fracture Liaison Services (FLSs)</a> and last year’s announcement of a <a href="https://www.osteoporos.org/ja-till-nytt-kunskapscentrum-for-osteoporos-i-stockholm/#:~:text=P%C3%A5%20ett%20m%C3%B6te%20i%20H%C3%A4lso,som%20f%C3%B6rebyggande%20%C3%A5tg%C3%A4rd%20mot%20sjukdomen." target="_blank">government-funded osteoporosis knowledge center in Stockholm, Sweden</a> – however, global efforts to tackle the treatment gap fall short of what is truly necessary. And, with the global population aging at a phenomenal rate, it is rapidly becoming a challenge we can no longer afford to ignore.</p><p><em>Closing the gaps in osteoporosis</em></p><p>As global leaders of the bone health community gather in London this week for the World Congress on Osteoporosis, Osteoarthritis and Musculoskeletal Diseases, we have a unique opportunity to confront some of the barriers that prevent us from the delivery of optimal patient care:<br><br><strong>Knowledge gap</strong>: IOF’s survey highlighted a disconnect between the occurrence of fractures amongst women at high-risk of osteoporosis and the decision by healthcare professionals to perform a diagnostic scan. And, as described by Kassim Javaid in the latest episode of the ½ðºÌÓéÀÖ³Ç podcast, Fragility Fractures in Focus, the lack of a recognized ‘bone specialism’ has resulted in a lack of clarity around which practitioners ‘own’ this area of medicine, and consequently, how it should be delivered effectively. How can we therefore equip ALL healthcare practitioners with the knowledge and the tools they need to ensure everyone with osteoporosis receives appropriate investigation and intervention after experiencing a fracture?<br><br><strong>Fragmented pathways</strong>: In most healthcare systems, patients who experience a fracture are often faced with disjointed pathways that are unable to deliver the necessary assessment for osteoporosis, once again, leading to missed opportunities for timely diagnosis, initiation of appropriate treatments and implementation of preventive measures to reduce the risk of subsequent fractures.</p><p><strong>Patient education and empowerment</strong>: Empowering patients with accurate information, clear guidance in treatment regimens and supporting resources can enable them to make informed decisions and take an active role in their care by having effective conversations with their doctors.</p><p><em>Post-fracture care – make or break?</em></p><p>Fortunately, a means to bridge these gaps already exists. Coordinated, multidisciplinary services such as FLSs enable rapid referral, facilitate early intervention and improve secondary fracture prevention. These services provide a seamless pathway post-fracture by systematically identifying patients, assessing their fall and fracture risk, and carefully evaluating the most effective intervention, placing the individual’s needs at the center of care. FLSs have been shown to be both clinically and cost-effective , and are described as the single most important step in directly improving patient care and reducing spiraling fracture-related healthcare costs worldwide.</p><p>But having a solution is only half the challenge. Global implementation of coordinated post-fracture services is dependent on the close collaboration between policymakers, payers, healthcare professionals and the pharmaceutical industry, and a unified commitment to elevate secondary fracture prevention as a healthcare priority.</p><p>We are proud to work closely with partners such as the Fragility Fracture Network and IOF, which look to make this vision a reality – IOF’s <a href="https://www.capturethefracture.org/" target="_blank">Capture the Fracture®</a> program recently celebrated a tremendous milestone of 900 registered FLSs worldwide, doubling the number of services since 2019. &nbsp;To support those looking to start a new FLS or improve the performance of an existing service, we and our partners at Amgen established the FLS Academy and Network (FAN), an educational program led by a Scientific Steering Committee of international experts. FAN offers healthcare professionals practical resources, expert guidance and access to knowledge-sharing events designed to help overcome the challenges faced by those on the ground.</p><p>The evidence is in front of us – we already have the tools at our disposal to close the treatment gap in osteoporosis. We’re excited to share and debate the very latest data and thinking, with the ultimate aim of delivering on FFN’s mission to optimise the multidisciplinary management of fragility fracture patients and the prevention of secondary fractures.&nbsp;<br> <br> This mission is very much in line with our own, which is why we’re so proud to work with FFN and other partners such as the International Osteoporosis Foundation through initiatives such as Capture the Fracture<sup>®</sup>, a global program to facilitate the implementation of Post-Fracture Care (PFC) Coordination Programs, such as Fracture Liaison Services (FLS), for secondary fracture prevention.<br> <br> Approximately 80% of individuals who have sustained a fracture due to osteoporosis do not receive the follow-up care they need, increasing the risk of suffering another costly and potentially life-altering break. FLSs provide routine assessment and management through a specialist multi-disciplinary team, thereby systematically reviewing patients’ risk for future fractures and their need for treatment, so they can receive optimal, personalised care. Coordinated PFC programs such as FLSs have been shown to improve outcomes of osteoporosis-related fractures and to reduce re-fracture rates and mortality, ultimately lessening the burden on healthcare systems.<br> <br> Such is their value, Japan and Denmark have recently made significant changes to their reimbursement schemes in the FLS space – changes which recognise the importance of PFC and which position FLSs as an essential part of the treatment pathway. In Japan, patients who have suffered a hip fracture will now be automatically assessed for osteoporosis in line with FLS clinical standards. In addition, the fee to manage secondary fracture prevention, which is often a significant barrier to care access, will also be reimbursed. In Denmark, FLSs will be reimbursed in four out of five regions over the course of 2022 – 2023.&nbsp;<br> <br> Spain, Italy and the UK have also recognised the urgency of dealing with the osteoporosis treatment gap; they are all actively investing in innovative new treatments and have updated guidelines to ensure patients can access to them as quickly as possible.<br> <br> There’s lots to be optimistic about, but always more we can do. As a result, osteoporosis is often overlooked – up to 80% of people who have suffered at least one fragility fracture are neither identified nor treated.&nbsp;Below, Professor Eugene McCloskey, Honorary Consultant and Professor in Adult Bone Disease at the University of Sheffield, shares his insights into how osteoporosis is identified and diagnosed.</p> <article data-quickedit-entity-id="media/43711" class="align-center media media--type-video media--view-mode-embedded"> <div class="field field--name-field-media-video-embed-field field--type-video-embed-field field--label-hidden field__items"> <div class="video-embed-field-provider-vimeo video-embed-field-responsive-video"><iframe width="854" height="480" frameborder="0" allowfullscreen="allowfullscreen" src="https://player.vimeo.com/video/759496502?autoplay=0"></iframe> </div> </div> </article> <p><br> Osteoporosis can have a crucial impact on a person’s life, as fragility fractures have the potential to impose a significant burden on quality of life, often making everyday activities such as eating, dressing, shopping or driving difficult.<br> <br> To mark this year’s World Osteoporosis Day on 20 October, ½ðºÌÓéÀÖ³Ç launched QUESTION FRACTURES MORE – a creative campaign designed to raise awareness of osteoporosis as the underlying cause of fragility fractures, and to inspire action amongst those at high-risk and their loved ones to help prevent future fractures.<br> <br> The campaign features a range of exciting, educational content across ½ðºÌÓéÀÖ³Ç.com and ½ðºÌÓéÀÖ³Ç social media channels, encouraging audiences to assess everyday scenarios that can lead to fragility fractures, and identify osteoporosis as the underlying cause. Despite taking the virtual stage, ASBMR remains to be a prominent centre point for the latest clinical and scientific developments in bone health. That is why we are excited to be taking part in this year’s congress, connecting with the osteoporosis community and hearing about the latest advances in bone health from around the world.<br><br>We believe that ongoing research is essential to keep the field moving toward optimal osteoporosis management and fracture prevention, but how much progress has been made with recent innovation in osteoporosis? A few years ago, we saw a welcome shift in the way osteoporosis is managed with the implementation of the Fracture Liaison Service (FLS). Best practices like the FLS have already undergone rigorous improvement over time, and their benefit is already widely acknowledged. More uptake of these services will provide solutions to the disconnect between patient and healthcare professional. <br><br>In addition to best practice processes like an FLS program, patients and physicians alike could benefit from a more individualized approach to treating and monitoring osteoporosis and fracture risk. In recent years, working groups and expert consensuses have expressed the potential benefit of a treat-to-target strategy to reduce the risk of fragility fractures. We can learn from other therapeutic areas which have benefited from a disease-specific treat-to-target approach to individualise the treatment of each patient based on their probability of reaching a treatment goal. These groups have commented that progress toward reaching the patient’s goal would include periodic and systematic assessment.<br><br>For physicians and their patients with osteoporosis, this could open up a new and refined approach to use existing therapies. Ideally, using biomarkers that are reliable, easily measured and cost effective and setting targets that are realistic, adaptable and have evidence of the benefits, could result in a strategy to help reduce fragility fracture risk while providing important near-term feedback to patients that their treatment is making a difference.<br><br>ASBMR will see the sharing of the latest clinical and scientific developments in bone health but we need health systems and policy makers involved in bringing these innovations to life, ensuring osteoporosis is no longer overlooked and under prioritised. While it is not the ‘live’ stage many had initially anticipated, the virtual congress is a fantastic opportunity to connect with the osteoporosis community and share insights into the latest advances in bone health from around the world. <br><br>In today’s current climate, the importance of continuing to connect from afar extends beyond congresses. Recently, ½ðºÌÓéÀÖ³Ç announced a new collaboration with the International Osteoporosis Foundation (IOF) to support its Capture the Fracture<sup>®</sup> program which aims to combat the global public health burden of osteoporosis by reducing the incidence of hip and vertebral fractures by 25% by 2025. <br><br>Osteoporosis is a serious chronic condition that weakens bone over time, making them thinner and more likely to break. We are fortunate though, that there are steps patients and healthcare providers can take to reduce fracture risk. Despite these resources, even after an osteoporosis-related fracture, approximately, 80% of individuals at high risk are still not identified or treated. Capture the Fracture, a global IOF initiative, now supported by ½ðºÌÓéÀÖ³Ç and Amgen and in collaboration with the University of Oxford, aims to proactively implement post-fracture care (PFC) coordination programs in hospitals and healthcare systems to help patients prevent subsequent fractures due to osteoporosis. <br><br>This partnership also welcomes collaboration from existing fracture prevention coalitions on international, regional, and national levels to drive fracture prevention policy change and prioritization. This can increase the risk of age-related conditions including osteoporosis. That’s why it’s time to do more to identify and manage those at risk of fragility fractures. <br><br></b></div><a href="https://www.healtheuropa.eu/osteoporosis-fragility-fractures-and-the-urgent-need-for-prioritisation-of-care/98206/" class target="_blank" id="ext-gen4171"> In the second of a series of thought leader articles by ½ðºÌÓéÀÖ³Ç personnel, in partnership with the Health Europa journal</a>, we look at the future of fracture prevention. It is a challenge that has evolved in recent decades as life expectancy has risen. <br><br>Between 2015 and 2030, the number of people in the world aged 60 years or over is projected to grow by 56 percent to 1.4 billion. By 2050, the global population of older people is projected to more than double its 2015 size, reaching nearly 2.1 billion. <br><br>This has significant implications age related conditions such as osteoporosis and fragility fractures. According to the International Osteoporosis Foundation (IOF), the increase in fragility fracture incidence across six European countries (France, Germany, Italy, Spain, Sweden and the UK) alone between 2017 and 2030 is estimated at 23 percent. <br><br>These fragility fractures have the potential to impose a significant burden on a person’s life, often making everyday activities such as eating, dressing, shopping or driving difficult. Those that suffer a hip fracture face reduced mobility, disability and loss of independence. In addition to the human cost, fragility fractures cost health systems in the European Union €37 billion in 2010 and based on demographic changes, costs are predicted to double by 2050. <br><br><b>Treat-to-target</b><br><br>One way to enhance and individualise the care of patients with osteoporosis would be a treat-to-target approach much in the same way that people with diabetes are often set an HbA1c target. Patients should be educated on their current disease status, in addition to being informed on future risks and their implications, at the time of diagnosis. <br><br>Additionally, Fracture liaison services (FLS) have been shown to reduce the frequency of subsequent fractures, improve adherence to treatment and provide cost savings when correctly implemented. But action is needed such that the care that FLS afford become the standard.<br><br>Ultimately, greater prioritisation needs to be given to osteoporosis identification, education and management. Following a hip fracture, 40% of people cannot walk independently and 80% cannot perform basic tasks such as shopping on their own. </div><br>To reduce the negative impact of fractures, Fracture Liaison Services (FLS) were introduced in the UK in the 1990s and are now offered in more than 35 countries. With demand for osteoporosis care rising as populations age, now is a good time to reflect on how the FLS model has changed patient care. In the first of a series of thought leader articles by ½ðºÌÓéÀÖ³Ç personnel, in partnership with the Health Europa journal, we look at the future of fracture prevention. <br><br>On the whole, the introduction of FLS has ushered in a period of significant progress in osteoporosis management. By coordinating care among multiple players, the frequency of subsequent fractures has been reduced and adherence to treatment has improved. The FLS approach also delivers cost savings – a vital feature of future osteoporosis services given the need for achieving sustainability while maintaining quality. <br><br>However, there is still room for improvement. Patient understanding of fracture risks can be low, with some attributing fractures to environment risks, random events or ageing. Healthcare professionals sometimes tend to take a passive approach to managing at-risk patients, rather than proactively engaging with women aged over 60, for example. <br><br>Awareness of, and referral to, FLS is also suboptimal. If patients with fragility fractures are not referred to FLS or secondary care setting, their risk of subsequent fractures is higher. Addressing this treatment gap is vital to reducing the burden on individuals and health systems. <br>&nbsp; <br>Greater public awareness of fragility fractures and their warning signs will go some way to raising these issues to national and global discussion. The research is part of our ongoing contribution to bring value to people affected by serious diseases, including osteoporosis.</div><div><br></div><div id="ext-gen1342">The results, based on a survey of 965 women over 60 with osteoporosis in 11 European countries, reveals considerable frustration among patients. Most respondents said they believe healthcare authorities are not doing enough to tackle fragility fractures and the devastating impact they can have on quality of life.</div><div><br> </div><div><b>Key findings:</b><br></div><ul><li id="ext-gen1351">65% said their condition is neglected</li><li>83% agreed that healthcare authorities should do more to prioritise osteoporosis</li><li>77% agrees that if more people aged under 60 suffered from osteoporosis it would be more of a priority</li><li id="ext-gen1272">58% said the impact of osteoporosis/fragility fractures is not fully understood or appreciated by their healthcare authority. </li></ul><div>The survey also highlighted the impact of the condition on quality of life. In addition to broken bones, patients reported that osteoporosis causes them pain (83%), makes them feel depressed (57%), vulnerable (61%), and isolated (27%).</div><div><br></div><div id="ext-gen1077">Of the women surveyed, over half said performing their usual daily activities (58%) and walking (56%) have become problematic due to their osteoporosis, and nearly a third (30%) said it impacted their ability to take care of themselves. </div><div><br></div><div>The condition can also affect patients’ ability to work. Almost one in five (17%) reported having to reduce their working hours, and one in 8 (13%) said the condition meant they had to give up work completely. </div><div><br></div><div id="ext-gen1359"><b>Experts agree: osteoporosis is overlooked</b></div><div id="ext-gen993">The findings echo concerns of leading experts in Europe. A <a href="/stories-media/press-releases/article/Experts-consider-osteoporosis-to-be-a-silent-epidemic-which-is-neglected-and-under-addressed-according-to-new-survey-released-today" class target="_blank">½ðºÌÓéÀÖ³Ç survey</a> of bone specialists published earlier this year found that 66% see osteoporosis as a neglected condition. Only 10% of specialists agreed that osteoporosis and fragility fractures are currently given a high priority by their local health authority.<br></div><div><br></div><div>While patients and clinicians believe osteoporosis is not given sufficient attention, the disease remains a major burden on individuals and health systems. It is the most common bone disease in the world, affecting around 200 million people globally. <br></div><div><br></div><div id="ext-gen1358">This number will rise as populations age. By 2050, the global population of older people is projected to more than double its 2015 size, reaching nearly 2.1 billion.
