What does the future look like for rare genetic epilepsies?

Research into rare genetic epilepsies is advancing rapidly, with multiple promising clinical developments that are underway. Across the industry, considerable efforts are being made to better care for people living with rare or treatment-resistant epilepsies through innovative science. At ��ֳ�, this means building a comprehensive understanding of the patient needs and the underlying causes of epilepsies, and seeking new ways to modify disease processes to improve outcomes.

Understanding the underlying disease mechanisms

Driving research into rare and treatment-resistant epilepsies requires us to combine our deep heritage in epilepsy with the latest technological innovations. By better understanding the molecular signatures and pathways that form the root causes of these complex diseases, it is opening up opportunities to target the underlying mechanisms that cause epilepsies.

KCNT1-related epilepsies are a group of rare epilepsies caused by a variation in the KCNT1 gene and include epilepsy of infancy with migrating focal seizures (EIMFS) and sleep-related hypermotor epilepsy (SHE). Most children have severe developmental and intellectual disability, and their seizures are difficult to control. ��ֳ� is collaborating with Praxis Precision Medicines, focusing on potential treatments for KCNT1-related epilepsies. This partnership aims to enhance understanding of the underlying causes of epilepsy, with a focus on exploring potential disease-modifying treatments.

It’s all part of our focus on disease modification, acting at the root cause of a disease rather than just addressing individual symptoms. By carefully observing the whole spectrum of symptoms and understanding of how epilepsies progress, coupled with research into the disease pathobiology, we hope to identify suitable targets for disease modification.

Beyond seizure control

The ultimate goal of our research is to find treatment approaches for people with rare genetic epilepsies who often struggle to find effective therapies and as a result, do not experience freedom from their persistent seizures. However, disease impact goes well beyond the symptomatic control of seizures and to make a meaningful impact on people’s lives we need to think more holistically about epilepsies.

For many people living with epilepsies, seizures are not always the primary or most impactful symptom they face in everyday life. Cognitive impairments, developmental delays, psychiatric conditions, and detrimental impacts on quality of life frequently accompany rare epilepsies yet are often underdiagnosed or inadequately treated. It’s essential that we consider these non-seizure outcomes associated with epilepsy as we seek to discover new solutions that look to improve quality of life holistically.

Several reports presented during this year’s American Epilepsy Society Congress (AES) focused on the relationship between sleep and epilepsy. A retrospective observational analysis found sleep apnea comorbidity is an important and potentially actionable factor associated with increased mortality in children and young adults with severe epilepsy. Traditionally sleep is assessed in a clinical setting using an elaborate electrical signal setup (EEG, ECG, EMG, EOG) * and respiration measurement. One study presented at AES found that sleep can be reliably assessed from a behind-the-ear EEG montage in an automated way in both healthy subjects and patients with Dravet Syndrome. The ability to assess sleep in a non-intrusive way has great potential to detect sleep-related co-morbidities associated with Dravet syndrome.

By better understanding the holistic challenges that the community faces we can drive advancements in how rare epilepsies are treated, with the full perspective of the patient and their family in mind.

A glimpse into the future

As we look to the future, it’s vital that we continue to raise awareness of rare epilepsies, partly due to the vast number of patients undiagnosed, but also to create a more supportive environment for all those living with these debilitating conditions.

The future of epilepsy research, particularly for rare genetic epilepsies, is promising. By focusing on new modalities, developing novel disease-modifying treatments, addressing comorbidities, and leveraging advanced technologies, we hope to find a way to not only control seizures but also improve overall quality of life for those living with epilepsies and their families. As our understanding of the epilepsy spectrum grows, so does the potential to explore new approaches that could change the course of rare epilepsies.

Today marked the start of the Liver Ring Trial, the first cross-pharma validation trial to validate the reproducibility and accuracy of a human liver Micro-Physiological System (MPS) in predicting drug-induced liver injury (DILI) and hepatic clearance. As one of the initiators of the Liver Ring Trial, we at ��ֳ� are thrilled to be a part of this pioneering initiative.

The trial leverages TissUse's groundbreaking organ-on-chip technology and esqLABS' advanced computational modelling to enhance drug safety assessment. The consortium aims to evaluate the robustness of MPS data across different laboratories and to provide the confidence to use MPS data to support decision making and, ultimately, regulatory submission. If successful, the effort should change the paradigm in non-clinical drug assessment from current simplified systems to complex in vitro models with improved translation to human studies.

The Liver Ring Trial will be conducted by six pharma companies across Europe (��ֳ�, Sanofi, AstraZeneca, Boehringer Ingelheim, Servier, Orion Pharma), TissUse (MPS platform provider), and ESQLabs (expert for AI/ML-based modelling and simulation solutions).

Guided by the support from the European Medicines Agency (EMA) and the European Center for the Validation of Alternative Methods (ECVAM), the Liver Ring Trial pioneers the use of MPS as the next level in preclinical drug assessment.

��ֳ� scientists, Benoit Cox (senior scientist, in vitro ADME), Andre Rodrigues (post-doc scientist, in vitro toxicology) and Reiner Class (Associate Director, in vitro ADME) have generated the scientific and strategic basis for the trial and will lead the consortium.

At ��ֳ�, by not being afraid to ask big questions, we are able to challenge the status quo and push the boundaries of medical knowledge, in our drive to make impactful changes to patients’ lives.

Creative and innovative thinking

The history of science is awash with examples of big questions that have led to remarkable leaps forward. For instance, asking the question what causes infectious diseases? led to the discovery of bacteria and viruses, and subsequently to the development of antibiotics and vaccines. We have come a long way since then, but we are still driving innovation and reaching for new discoveries by probing the unknown with big questions. These questions require creative and innovative thinking, and they challenge us to explore new areas of inquiry that were previously unexplored.

Here at ��ֳ�, we see ‘innovation’ as inventing something new and useful. And in no field is that more relevant than in that of medicine – something ‘new’ could be a disease modifying medicine, and something ‘useful’ could be the novel way in which we discover it.

Innovative digital technology is undoubtedly transforming how we treat and manage disease. This is in large part down to Artificial Intelligence (AI) and data generation, processing and computing speeds, which make the impossible, possible. For example, AI can help describe the structural connections between atoms and molecules and optimize desirable properties for drugs. We can now sequence the hundreds of amino acids in each and every protein. Then, informed by AI, we can edit antibody sequences to be more stable and effective against disease-causing proteins. AI can also interpret the health of individual cells and - using this information - we can gain a deeper understanding of disease biology and pathology, and how drugs may be able to reverse it.

At the clinical trials stage, advanced AI can help to keep track of treatment schedules and trial recruitment, help to make trials easier to access for patients, and make the data easier to access for physicians. This is all a work in progress, we are constantly ‘training’ and advancing the capabilities of our AI models, with each new discovery and input of data, AI is learning and getting more efficient.

But the value of AI for patients, now and in the future, is undeniable –the more efficient use of genetic data, and the modelling of disease pathways and gene sequences all help us to identify exactly what the patient needs so that we can works towards individualise treatment. It’s also removing the barriers imposed by ethnicity or geography, because we now have access to, and an understanding of, much larger data sets.

AI shows us not only the power of digital technology but also the power of collaboration – our chemists and biologists are working side by side and trusting our data scientists to empower scientific research.

AI is not without its challenges of course – it is difficult to define a ‘gold standard’ for the pharma domain, particularly given the rate of progress, so there is a test-and-learn approach which must be adopted when analysing and revaluating our AI models. Attention must be paid to any bias embedded in AI frameworks, making sure that they are generated with comprehensive and 'clean' data.

Ultimately though, we believe that innovations like AI will help enable new discoveries and accelerate drug development, all for the benefit of people suffering from disease. That’s why innovation is our driving force at ��ֳ�. We're filled with anticipation about what lies ahead!

While both important, at ��ֳ� we believe that ‘innovation’ and ‘invention’ are quite distinct. An invention is something completely new, but innovation can be more than just a single idea; it can be found in small, incremental changes or improvements to how we work, how we think, how we see or how we use things, or to pre-existing systems, hierarchies, models, products, services, and processes. But when taken together, these small changes - or improvements – made to an existing product or service can make a big difference and add value.

Examples of incremental innovation are everywhere. The laser, for example, was invented in 1958 and first applied in healthcare as a non-contact scalpel. New applications of lasers include reshaping corneas, photodynamic therapy for cancer, and transmyocardial revascularization (TMR) for severe angina. Even Apple, famed for disruptive innovation, take an incremental innovation approach to its iPhone. It has maintained releasing regular upgraded versions; each offering small scale improvements.

We live in a world that likes ‘quick-wins’ and short-cuts. However, in science, making advancements simply does not happen that way. It takes dedication, hard work, and often time. So, at ��ֳ� we celebrate the small improvements and recognise their value as steps forward in a continuous pursuit of innovation as we strive to improve the lives of people suffering severe diseases.
